      缺血性脑卒中现已成为我国中老年人残疾和死亡的主要原因之一,卒中后多领域功能障碍的恢复已经成为康复领域关注热点。重复经颅磁刺激(repetitive transcranial magnetic stimulation, rTMS)作为一种无创、安全的神经调控技术已经广泛应用于神经精神疾病且取得良好临床疗效。rTMS常见模式包括低频重复经颅磁刺激、高频重复经颅磁刺激及Theta爆发式磁刺激(theta-burst stimulation, TBS)。TBS涵盖两种刺激模式:具有兴奋作用的间歇性θ短阵脉冲刺激(intermittent TBS,iTBS)和具有抑制作用的持续性θ短阵脉冲刺激(continuous TBS,cTBS)。TBS能在较短时间内达到治疗效果,其安全性及有效性已经得到临床验证。然而对于TBS治疗缺血性脑卒中的机制尚未明确,因此本文总结近年TBS治疗缺血性脑卒中的机制研究内容,探讨TBS调节卒中后神经元功能及改善卒中结局的可能机制。TBS可应用于卒中后运动障碍、痉挛、认知障碍、失语、忽视等多种卒中后功能障碍,本文简要概述目前TBS临床研究进展及常见有效治疗方案,为临床应用提供参考。

      Ischemic stroke has become one of the main causes of disability and death among middle-aged and elderly people in China, and the recovery of post-stroke dysfunction has become a hotspot in the field of rehabilitation. As a non-invasive and safe neuromodulation technique, repetitive transcranial magnetic stimulation (rTMS) has been widely used in neuropsychiatric disorders and has achieved good clinical efficacy. rTMS commonly includes low-frequency repetitive transcranial magnetic stimulation, high-frequency repetitive transcranial magnetic stimulation, and theta-burst stimulation TMS. TBS consists of two stimulation modes: intermittent TBS (iTBS) with excitatory effect and continuous TBS (cTBS) with inhibitory effect. TBS can achieve therapeutic effect in a relatively short period of time, and its safety and efficacy have been clinically proven. However, the mechanism of TBS for ischemic stroke has not yet been clarified, therefore, this article summarizes the recent studies on the mechanism of TBS for ischemic stroke, and explores the possible mechanisms by which TBS regulates neuronal function and improves the outcome of stroke.TBS can be applied to a variety of post-stroke dysfunctions such as post-stroke dyskinesias, spasticity, cognitive deficits, aphasia, and neglect, etc. This article briefly summarizes the progress of the clinical studies and the common effective treatment options of TBS, and provides a brief overview of the clinical progress and the common effective treatment options.
