目的 探究N-myc下游调控基因2(NDRG2)、哺乳动物雷帕霉素靶蛋白(mTOR)与缺血性脑卒中患者美国国立卫生研究院卒中量表(NIHSS)评分的相关性及其联合预测临床预后的价值。方法 选取2020年6月~2021年12月我院300例缺血性脑卒中患者作为研究对象,根据患者发病后90 d改良Ranking量表(mRS)评分分组,其中mRS评分0~1分为无残疾组(n=153),2~5分为残疾组(n=147)。比较两组临床资料、NIHSS评分、NDRG2、mTOR蛋白,相关性分析NDRG2、mTOR蛋白与NIHSS评分相关性,绘制受试者工作(ROC)曲线,评价NDRG2、mTOR蛋白、NIHSS评分对缺血性脑卒中患者临床预后的预测价值,并以Logistic回归分析NDRG2、mTOR蛋白、NIHSS评分对缺血性脑卒中患者临床预后的关系。结果 残疾组NIHSS评分、NDRG2、mTOR蛋白表达高于无残疾组,差异有统计学意义(P<0.05);相关性分析可知,缺血性脑卒中患者NDRG2、mTOR蛋白表达与NIHSS评分均呈正相关关系(P<0.05);绘制ROC曲线,评价NIHSS评分、NDRG2、mTOR蛋白表达对缺血性脑卒中患者临床预后的预测价值,显示三者联合预测AUC最大,具有良好预测效能;Logistic回归分析,结果可知NDRG2、mTOR蛋白、NIHSS评分均为缺血性脑卒中患者临床预后影响因素,且NDRG2、mTOR蛋白、NIHSS评分越高,患者残疾风险越高(P<0.05)。结论 缺血性脑卒中患者NDRG2、mTOR蛋白表达与NIHSS评分呈正相关,且可为该病患者临床预后影响因素,临床监测二者水平,指导缺血性脑卒中防治工作,有助于改善患者预后。 |

Objective To explore the correlation between NDRG2, mTOR protein and the National Institutes of Health Stroke Scale (NIHSS) scores in patients with ischemic stroke and the value of their combination in predicting clinical prognosis. Methods A total of 300 patients with ischemic stroke in our hospital from June 2020 to December 2021 were selected as the research objects. The patients were grouped according to the modified Ranking Scale (mRS) score 90 days after the onset of the disease. The higher the NDRG2, mTOR protein, and NIHSS scores, the higher the risk of poor clinical prognosis in the near future (P<0.05). Conclusion The expression of NDRG2 and mTOR protein in patients with ischemic stroke is positively correlated with NIHSS scores, and may be factors influencing the clinical prognosis of patients with the disease.
