      【摘要】 目的 探索体外冲击波治疗脑卒中后下肢痉挛的疗效。 方法 计算机检索PubMed、Cochrane Library、Embase、CNKI、WanFang Data、CBM,搜集体外冲击波治疗脑卒中后下肢痉挛的随机对照试验,检索时限为建库至2023年5月。以改良Ashworth量表(MAS)、Fugl-Meyer评定量表(FMA)、关节活动度(ROM)为结局指标,用Cochrane手册进行偏倚风险评估,采用RevMan 5.4软件进行meta分析。 结果 最终纳入11篇文献,共508例患者。分析结果显示,与对照组相比,干预组患者MAS [MD=-0.32,95%CI( -0.41,-0.23),p<0.05]、 FMA [MD=2.05,95%CI(1.32,2.78),p<0.05]、ROM [MD=1.69,95%CI(0.52,2.86),p<0.05]有显著改善,差异有统计学意义。 结论 当前证据显示,体外冲击波对脑卒中后下肢痉挛具有积极作用。此外,没有观察到任何患者在冲击波治疗后的严重副作用。

      【Abstract】 Objective To explore the effect of extracorporeal shock wave therapy on lower limb spasm after stroke. Methods PubMed, Cochrane Library, Embase, CNKI, WanFang Data and CBM were searched by computer to collect randomized controlled trials of extracorporeal shock wave therapy for lower limb spasm after stroke. The search time was from the establishment of the database to May 2023. Modified Ashworth Scale (MAS), Fugl-Meyer Rating Scale (FMA) and range of motion (ROM) were used as outcome indicators. The risk of bias was assessed using Cochrane manual and meta-analysis was performed using RevMan 5.4 software. Results Eleven randomized controlled trials with a total of 508 patients were included. The analysis results showed that compared with the control group, patients in the intervention group had improved Ashworth scale [MD=-0.32,95%CI(-0.41,-0.23),p < 0.05] and Fugl-Meyer rating scale [MD=2.05, 95%CI(1.32,2.78),p < 0.05] and joint motion [MD=1.69, 95%CI(0.52,2.86),p < 0.05] were significantly improved, and the difference was statistically significant. Conclusions Current evidence shows that extracorporeal shock wave has a positive effect on lower limb spasm after stroke. In addition, no serious side effects were observed in any of the patients after shockwave therapy.
