      【摘要】 目的:探讨体外冲击波对脑卒中患者手部肌张力增高的治疗效果观察。 方法:选取60名符合实验要求的脑卒中患者,利用Excel软件进行随机分组,对照组(n=30)和实验组(n=30),对照组进行常规的物理疗法、作业疗法、理疗、中医康复等康复治疗方法,实验组(n=30)除上述康复治疗外,每周进行2次的体外冲击波治疗,强度0.8~1.5bar,频率10Hz,治疗持续4周。两组患者分别在治疗前后对患者进行上肢运动功能(Fugl-Meyer assessment,FMA)评定,手部的改良Ashworth(modified Ashworth scale,MAS)肌张力评定(手部屈肌肌群评定),上肢及手功能 DASH(Disabilities of the Arm,Shoulder,and Hand questionnaire,DASH)问卷和日常生活能力改良Barthel指数(modified Barthel index,MBI)评定以及患者对此次治疗的满意度评定。 结果:治疗4周后,两组患者在上肢运动功能,手部肌张力,上肢及手功能DASH问卷以及日常生活能力的评定均有所改善(P<0.05),实验组优于对照组(P<0.05),实验组获得治疗的满意度高于对照组(P=0.028). 结论:体外冲击波在一定程度上能维持和改善脑卒中患者手部屈肌肌张力增高的现象,同时有助于提高患者上肢运动机能及手功能,提高患者日常生活能力,并获得患者对治疗的满意评价。

      【 abstract 】 Objective: To observe the therapeutic effect of extracorporeal shock wave (ESWL) on increased hand muscle tension in stroke patients. Methods: 60 stroke patients who met the experimental requirements were selected and randomly divided into control group (n=30) and experimental group (n=30) using Excel software. The control group received conventional physical therapy and occupational therapy physiotherapy In the experimental group (n=30), in addition to the above-mentioned rehabilitation treatment, extracorporeal shock wave therapy, intensity of 0.8~1.5bar, frequency of 10Hz, was performed twice a week for 4 weeks in the experimental group (n=30). Patients in the two groups received upper limb motor function before and after treatment (fugl-meyer) Ashworth(Modified Ashworth Scale,MAS) muscle tone assessment, DASH(Disabilities of the Arm,Shoulder,and Hand) Questionnaire,DASH), Modified Barthel Index (MBI), and patients' satisfaction with the treatment. Results: After 4 weeks of treatment, both groups showed improvement in upper limb motor function, hand muscle tone, DASH questionnaire of upper limb and hand function, and assessment of daily living (P<0.05), the experimental group was superior to the control group (P<0.05), the satisfaction of treatment in the experimental group was significantly higher than that in the control group (P=0.028). Conclusion: To a certain extent, extracorporeal shock wave can maintain and improve the phenomenon of increased hand muscle tension in stroke patients, and at the same time help to improve the upper limb motor function and hand function of patients, improve the ability of daily living of patients, and obtain patients' satisfactory evaluation of treatment.
