      目的:本研究使用功能性近红外光谱成像(functional near-infrared spectroscopy,fNIRS)分析中风后亚急性期命名性失语患者的静息状态和图片命名过程中的皮层激活模式。 方法:研究纳入了15名中风后亚急性期命名性失语患者。同时招募了20名年龄和性别匹配的健康受试者作为对照组。在静息态和图片命名任务中收集fNIRS数据,并选择相关语言脑区以分析大脑激活情况。 结果:在静息状态下,与对照组相比,命名性失语组的左侧上边缘回(supramarginal gyrus ,SMG)和右侧颞上回(superior temporal gyrus ,STG)的氧合血红蛋白浓度降低(P<0.05),并且在左侧SMG-右侧Broca区和左侧SMG-右侧颞中回(middle temporal gyrus ,MTG)的功能连接增强(P<0.05)。在图片命名任务期间,与对照组相比,命名性失语组在左侧辅助运动区(supplementary motor cortex ,SMA)、左侧角回(angular gyrus ,AG)以及左侧和右侧SMG脑区的激活降低(P<0.05)。此外,图片命名期间左侧SMG脑区的β值与图片命名任务的得分呈显著正相关(P<0.05)。 结论:本研究通过fNIRS发现,左侧SMG在命名性失语患者中激活显著降低,且其激活水平与图片命名任务表现正相关,支持其在语言障碍中的关键作用及作为康复治疗靶点的潜力。

      Objective: This study used functional near-infrared spectroscopy (fNIRS) to analyze the cortical activation patterns in subacute post-stroke anomic aphasia patients during rest and picture naming tasks. Method: The study included 15 subacute post-stroke anomic aphasia patients and 20 age- and gender-matched healthy subjects as a control group. fNIRS data were collected during rest and picture naming tasks, and relevant language brain areas were selected to analyze brain activation. Results: During the resting state, compared with the control group, the oxygenated hemoglobin concentration in the left supramarginal gyrus (SMG) and right superior temporal gyrus (STG) was reduced in the anomic aphasia group (P<0.05), and the functional connectivity between the left SMG-right Broca's area and left SMG-right middle temporal gyrus (MTG) was enhanced (P<0.05). During the picture naming task, the activation in the left supplementary motor cortex (SMA), left angular gyrus (AG), and left and right SMG was lower in the anomic aphasia group compared with the control group (P<0.05). Moreover, the β values in the left SMG region during the picture naming task were significantly positively correlated with the scores on the picture naming task (P<0.05). Conclusion: In this study, we utilized functional near-infrared spectroscopy and identified that the activation of the left SMG was significantly diminished in patients with anomic aphasia. Moreover, the activation level of the left SMG was positively correlated with performance in the picture naming task. These findings collectively support the critical role of the left SMG in language impairment and highlight its potential as a therapeutic target for rehabilitation.
