| 目的 采用Meta分析的方法对认知-运动双任务训练对于脑卒中后患者的认知功能的改善效果进行分析。方法 计算机检索 CNKI、万方、维普、中国生物医学文献数据库(Sinomed)、PubMed、Web of Science 等国内外数据库(建库至2024 年 10 月)中相关临床随机对照试验(RCT)。使用软件RveMan5. 4.1进行Meta分析,通过对主要结局指标:简易精神状态检查量表(MMSE),蒙特利尔认知评估量表(MOCA),连线测试A(TMT-A),及次要结局指标:改良Bathel指数(MBI),评价脑卒中患者经认知-运动双任务训练后,其认知功能的改善效果。结果 共计12篇学术文献纳入研究,其中包括中文研究文献10篇,外文研究文献2篇,总样本量共1032例。Meta分析结果经治疗后试验组MMSE量表与MOCA量表评分增加,整体认知功能显著改善[(WMD=2.55,95% CI:1.35~3.75,P<0.01)][(WMD=3.28,95% CI:2.25~4.32,P<0.01)]。试验组TMT-A量表评分降低,执行功能改善显著[( WMD=-9.95,95% CI:-12.35~-7.55,P<0.05)]。试验组MBI量表评分增加,日常生活能力改善显著[(WMD=12.58,95% CI:10.31~14.86,P<0.01)]。结论 认知-运动双任务训练对于脑卒中后患者认知功能的影响有一定效果,该疗法可在临床推广。 |

| The aim of this research is to access the improvement impact of cognitive-motor dual-task training for the cognitive abilities of post-stroke patients by meta-analysis. Clinical randomized controlled trials(RCTs) about the impact of cognitive-motor dual-task training for cognitive abilities of stroke patients were searched for domestic and foreign databases such as CNKI, Wanfang Database, VIP, Chinese Biomedical Literature Database, PubMed, and Web of Science by computer. The search time was from the establishment of the database to October 2024. Meta-analysis was performed using software RevMan5.4.1. The main outcome measures were the Mini-Mental State Examination Scale (MMSE), the Montreal Cognitive Assessment Scale (MOCA), and the Trail Making Test A (TMT-A). The secondary outcome measure was the Modified Barthel Index (MBI). The enhancing impact of cognitive-motor dual-task training on the cognitive abilities of stroke patients was evaluated. Results: The study encompassed a total of 12 pieces of literature, consisting of 10 Chinese literatures and 2 Foreign literatures, and the overall sample size amounted to 1032 cases. The results of meta-analysis showed that after treatment the scores of MMSE scale and MOCA scale in the experimental group increased, and the overall cognitive function was significantly improved [(WMD = 2.55, 95% CI: 1.35 - 3.75, P < 0.01)][(WMD = 3.28, 95% CI: 2.25 - 4.32, P < 0.01)]. The score of TMT-A scale in the experimental group decreased, and the executive abilities was significantly improved [(WMD = -9.95, 95% CI: -12.35 - -7.55, P < 0.05)]. The score of MBI scale in the experimental group increased, and the ability of daily living was significantly improved [(WMD = 12.58, 95% CI: 10.31 - 14.86, P < 0.01)]. Conclusion: Cognitive-motor dual-task training has a positive effect on the cognitive function of post-stroke patients, and this therapy is worthy of clinical promotion. |
