白细胞介素-6(Interleukin-6,IL-6)通过免疫和炎症机制,不仅在癫痫的发生与发展中发挥了重要作用,而且同时参与了癫痫相关精神心理并发症的病理生理过程。IL-6可能导致癫痫患者的情绪调节异常和认知功能障碍,并可能与癫痫共患抑郁障碍的发病机制密切相关。本综述旨在探讨IL-6在癫痫及其共患抑郁障碍中的双重作用,强调其作为潜在治疗靶点和连接癫痫和抑郁障碍中桥梁的意义。本文系统分析了IL-6在原发性癫痫、自身免疫性癫痫、新发难治性癫痫持续状态、癫痫共病抑郁以及神经精神合并症中的作用机制和研究进展。同时,针对以IL-6为靶点的药物治疗现状进行总结与评估。提出未来研究应重点阐明IL-6在癫痫不同亚型、疾病阶段及抑郁共病中的具体作用机制的理论观点,从而为制定更精准、更高效的个体化诊疗策略提供依据。此外,IL-6有望成为特定类型癫痫早期诊断和预后评估的生物标志物,其应用潜力亦值得进一步深入探索。 |

Interleukin-6 (IL-6), through immune and inflammatory mechanisms, plays a crucial role not only in the onset and progression of epilepsy but also in the pathophysiology of epilepsy-related psychiatric and psychological comorbidities. IL-6 may contribute to emotional dysregulation and cognitive impairment in epilepsy patients and is potentially involved in the pathogenesis of comorbid depressive disorders. This review aims to explore the dual role of IL-6 in epilepsy and epilepsy-associated depressive disorders, emphasizing its significance as a potential therapeutic target and a bridge linking epilepsy and depressive conditions. The article systematically analyzes the mechanisms and research progress of IL-6 in primary epilepsy, autoimmune epilepsy, new-onset refractory status epilepticus, epilepsy comorbid with depression, and neuropsychiatric complications. Additionally, it summarizes and evaluates the current status of IL-6-targeted pharmacological therapies. Future research should prioritize elucidating the specific mechanisms of IL-6 in different epilepsy subtypes, stages of the disease, and comorbid depressive disorders. This understanding will provide a theoretical basis for developing more precise and effective individualized treatment strategies. Furthermore, IL-6 holds promise as a biomarker for early diagnosis and prognosis assessment in specific types of epilepsy, warranting further exploration of its clinical application potential. |
