| 目的 探讨经口间歇管饲(IOE)与神经肌肉电刺激治疗脑卒中后吞咽障碍患者的效果,及其对吞咽功能、机体营养状况的影响。方法 选用本院收治的卒中后吞咽障碍患者92例,随机分为联合方案组、对照组。对照组给予常规留置鼻饲管进食和神经肌肉电刺激,联合方案组给予神经肌肉电刺激和IOE进食。比较两组患者标准吞咽功能评价量表(SSA)评分、洼田饮水试验等级、总有效率及相关营养指标水平。结果 经过治疗后,联合方案组SSA评分显著降低,低于对照组(P<0.05);洼田饮水试验分级明显改善,优于对照组(P<0.05)。联合方案组总有效率(93.48%)较对照组(78.26%)明显提高(P<0.05)。治疗后,两组血清白蛋白(ALB)、血红蛋白(Hb)、血清前白蛋白(PA)、血清转铁蛋白(sF)、总蛋白(TP)含量明显升高(P<0.05),且联合方案组较对照组明显更高(P<0.05);两组吸入性肺炎、呕吐等发生率无显著差异(P>0.05)。结论 对脑卒中后吞咽障碍患者采用IOE+神经肌肉电刺激治疗,效果较为理想,临床应用价值较高。 |

| Objective To explore the therapeutic effect of intermittent oro-esophageal tube feeding (IOE) combined with neuromuscular electrical stimulation on post-stroke dysphagia, and its effects on swallowing function and nutritional status of the body. Methods Ninety-two patients with dysphagia after stroke in our hospital were randomly divided into combination program group and control group. The control group received conventional indwelling nasal feeding tube feeding and neuromuscular electrical stimulation treatment, while the combined scheme group received IOE feeding and neuromuscular electrical stimulation treatment. The SSA scores, water swallow test grading, total effective rate, and related nutritional indicators of the two groups were compared. Results After treatment, the SSA score of the combined regimen group obviously decreased, and was lower than that of the control group (P<0.05); the grading of the water swallow test obviously improved, and was better than control group (P<0.05). The total effective rate of the combined regimen group (93.48%) was obviously higher than that of the control group (78.26%) (P<0.05). After treatment, the levels of hemoglobin (Hb), serum albumin (ALB), serum transferrin (sF), serum prealbumin (PA), and total protein (TP) in the two groups were obviously increased (P<0.05), and the levels in the combined scheme group were obviously higher than those in the control group (P<0.05); there were no significant differences in the incidence of aspiration pneumonia and vomiting between the two groups (P>0.05). Conclusion The combined treatment of IOE and neuromuscular electrical stimulation for patients with post-stroke dysphagia has ideal effect and high clinical application value. |
