目的 探究经颅黑质超声(TCS)联合血清壳多糖酶3样蛋白1(CHI3L1)、富亮氨酸重复激酶2(LRRK2)对帕金森病(PD)的诊断价值。方法 选取我院2020年6月~2023年5月期间收治的72例PD患者作为研究组,根据Hoehn-Yahr分级评估患者疾病严重程度,分为早期组24例,中期组32例和晚期组16例,另选取同期收治的85例非PD精神疾病患者作为疾病对照组,72例体检者作为健康对照组。采用Pearson法分析血清CHI3L1、LRRK2表达水平与黑质(SN)强回声面积的相关性;采用受试者工作特征(ROC)曲线分析血清CHI3L1、LRRK2及SN强回声面积对早期PD的诊断价值。结果 研究组与疾病对照组、健康对照组的SN强回声面积、血清CHI3L1及LRRK2表达水平存在显著性差异(P<0.05);PD患者SN强回声面积、血清CHI3L1及LRRK2表达水平均随病情严重程度的增加而升高(P<0.05);PD患者血清CHI3L1、LRRK2表达水平均与SN强回声面积呈正相关(r=0.638、0.691,P均<0.001);早期PD患者与对照组SN强回声面积、血清CHI3L1及LRRK2表达水平存在显著性差异(P<0.05);SN强回声面积、血清CHI3L1及LRRK2诊断早期PD的AUC分别为0.724、0.694及0.657,而三者联合检测诊断的AUC为0.850,优于SN强回声面积、血清CHI3L1及LRRK2各自单独诊断(Z三者联合-SN强回声面积=2.350、Z三者联合-CHI3L1=2.656、Z三者联合-LRRK2=2.414,P均<0.05)。结论 PD患者血清CHI3L1和LRRK2表达水平与PD的发生及发展有关,二者联合TCS检查对早期PD具有较高的诊断价值。 |

Objective: To investigate the diagnostic value of transcranial sonography (TCS) combined with serum chitinase-3-like protein 1 (CHI3L1) and leucine rich repeat kinase 2 (LRRK2) in Parkinson"s disease (PD). Methods From June 2020 to May 2023, 72 PD patients admitted to our hospital were collected as the study group, according to the Hoehn-Yahr grading, patients were grouped into an early group of 24 cases, an intermediate group of 32 cases, and an advanced group of 16 cases, 85 non PD mental illness patients admitted during the same period were selected as the benign control group, and 72 physical examination subjects were selected as the blank control group. Pearson method was applied to analyze the correlation between the expression levels of serum CHI3L1 and LRRK2 and the strong echo area of substantia nigra (SN); receiver operating characteristic (ROC) curve was applied to analyze the diagnostic value of serum CHI3L1, LRRK2, and SN strong echo area for early PD. Results There were significant differences (P<0.05) in the strong echo area of SN, serum CHI3L1 and LRRK2 expression levels between the study group and the benign and blank control groups; the strong echo area of SN and the expression levels of serum CHI3L1 and LRRK2 in PD patients increased with the aggravation of disease (P<0.05); the expression levels of serum CHI3L1 and LRRK2 in PD patients were positively correlated with the strong echo area of SN (r=0.638, 0.691, P<0.001); there were significant differences (P<0.05) in the SN hyperechoic area, serum CHI3L1, and LRRK2 expression levels between early PD patients and the control group; The AUC of SN hyperechoic area, serum CHI3L1, and LRRK2 for the diagnosis of early PD were 0.724, 0.694, and 0.657, respectively, while the AUC of the combined detection of the three was 0.850, which was better than the individual diagnosis of SN hyperechoic area, serum CHI3L1, and LRRK2 (Z three combined - SN hyperechoic area=2.350, Z three combined - CHI3L1=2.656, Z three combined - LRRK2=2.414, all P<0.05). Conclusion The expression levels of CHI3L1 and LRRK2 in serum of PD patients are related to the occurrence and development of PD, and the combination of them and TCS examination has high diagnostic value for early PD. |
