目的 探讨本体感觉神经肌肉促进技术(PNF)联合运动想象疗法在脑卒中后偏瘫患者中的应用效果。方法 选取2019年12月~2022年12月我院104例脑卒中后偏瘫患者,随机分为两组,各52例。两组均予以常规康复训练,于此基础上,对照组予以运动想象疗法,观察组予以PNF配合运动想象疗法。对比两组临床疗效、改良Barthel指数(MBI)、Fugl-Meyer下肢功能量表(FMA-LE)评分、端点行程、步速、移动速度、步幅、反应时间、步频、膝关节与髋关节屈伸平均角速度及股四头肌、胫骨前肌、腓肠肌、股二头肌iEMC值。结果 观察组临床治疗总有效率90.38%(47/52)高于对照组73.08%(38/52)(P<0.05);治疗1个、2个疗程观察组MBI评分、FMA-LE评分均高于对照组(P<0.05);观察组治疗1个、2个疗程反应时间短于对照组,端点行程、移动速度步速、步幅、步频及髋关节、膝关节屈伸平均角速均大于对照组(P<0.05);治疗1个、2个疗程观察组股二头肌、股四头肌、胫骨前肌、腓肠肌iEMC值大于对照组(P<0.05)。结论 PNF联合运动想象疗法应用于脑卒中后偏瘫患者中疗效良好,可增强下肢肌群肌力,改善步态特征,恢复平衡能力,提升肢体功能,从而增强患者生活自理能力。 |

Objective To investigate the effect of proprioceptive neuromuscular facilitation (PNF) combined with motor imagery therapy in patients with hemiplegia after stroke. Methods A total of 104 patients with hemiplegia after stroke in our hospital from December 2019 to December 2022 were randomly divided into two groups with 52 cases in each group. Both groups were given routine rehabilitation training. On this basis, the control group was given motor imagery therapy, and the observation group was given PNF combined with motor imagery therapy. The clinical efficacy, modified Barthel index (MBI), Fugl-Meyer Lower Limb Function Scale (FMA-LE) score, endpoint length, walking speed, moving speed, stride length, reaction time, stride frequency, average knee and hip flexion and extension angular velocity, and iEMC values of quadriceps, tibialis anterior muscle, gastrocnemius muscle and biceps femoris were compared between the two groups. Results The total effective rate of observation group was 90.38% (47/52) higher than that of control group 73.08% (38/52) (P < 0.05). MBI score and FMA-LE score in observation group were higher than those in control group after 1 and 2 treatment courses (P < 0.05). The reaction time of observation group after 1 and 2 treatment courses was shorter than that of control group, and the endpoint stroke, moving speed, stride length, stride frequency and average Angle velocity of hip joint and knee joint flexion and extension were all greater than those of control group (P < 0.05). The iEMC values of biceps femoris, quadriceps femoris, tibialis anterior muscle and gastrocnemius in observation group were higher than those in control group (P < 0.05). Conclusion PNF combined with motor imagery therapy has good efficacy in patients with hemiplegia after stroke, which can enhance the muscle strength of lower limb muscle group, improve gait characteristics, restore balance ability, improve limb function, and thus enhance the patients" self-care ability. |
