      目的:评估3D打印导板导航技术应用于三叉神经半月神经节射频热凝术(PRT)的临床疗效。方法:选取我院35例原发性三叉神经痛,有PRT指征的患者为研究对象,随机分为观察组(n=20)和对照组(n=15),观察组用3D打印导板导航辅助PRT,对照组在DSA引导下行PRT。比较两组经皮穿刺到达靶点所需时间、一次穿刺成功率、手术时长、DSA扫描次数、患者术中不适、术后即刻、术后3天、30天疼痛评分、术后1、3、6月疗效评估、手术满意度、并发症发生率。结果:观察组在一次穿刺成功率、穿刺到达靶点时间、手术时长、DSA扫描次数、术中不适、术后不良反应、术后即刻、术后3天疼痛评分均低于对照组(P<0.001)。两组术后30天疼痛评分、术后1、3、6月疗效评估、满意度、二次手术差异无统计学意义(P>0.05)结论:3D打印导板导航技术辅助PRT可提高手术的高效性、安全性,减少疼痛及不良反应,有良好临床应用前景。

      Abstract Objective: To evaluate the clinical efficacy of 3D printed guide navigation technology in percutaneous radiofrequency thermocoagulation of trigeminal semilunar ganglion (PRT).Methods: Thirty-five hospitalized patients with trigeminal neuralgia were randomly divided into observation group (n = 20) and control group (n = 15), Observation group patients were treated under the guidance of navigation system. The control group was treated by DSA guide. The time of puncture to target, success rate of one-time puncture, operation duration, the number of DSA scans,the pain scores immediately after surgery, 3 days,30 days after surgery and clinical efficacy at 1m, 3m, and 6m,satisfaction of operation as well as the incidence of complications and adverse reactions were compared between the two groups. Results: Success rate of one puncture,time of puncture to target,operation duration, DSA scan times,incidence of intraoperative and postoperative adverse reactions, pain scores immediately after surgery and 3 days after surgery in the observation group were significantly reduced compared with control group(P<0.001). There was no significant difference in pain scores at 30 days after surgery, evaluation of efficacy, satisfaction and second operation 1, 3 and 6 months after surgery between the two groups (P>0.05).Conclusion: 3D printing guide navigation technology to assist PRT can improve the efficiency and safety of surgery, reduce pain and adverse reactions, and has a good clinical application prospect.
