      目的:研究人际心理技术(IPT)抗抑郁治疗的作用,并通过氢质子磁共振波谱(1 H-MRS)检测初步探 索其机制。方法:抑郁症患者17例纳入病例组,同期健康体检志愿者17例纳入对照组。抑郁症患者予以 IPT 治疗 4 周。于治疗前、治疗第 28 天时,采用阳性与阴性症状量表(PANSS)、汉密尔顿抑郁量表 (HAMD)-24 进行疗效评估。行海马 1 H-MRS 扫描,分析 2 组及病例组治疗前、后的 N-乙酰天门冬氨酸 (NAA)/[胆碱(Cho)+肌酸(Cr)]、NAA/Cr、NAA/Cho和Cho/Cr比值。结果:病例组患者治疗后PANSS阴性 症状、一般情况及总分、HAMD-24评分较治疗前明显降低(P<0.01),PANSS阳性症状较治疗前差异无统计 学意义(P>0.05)。治疗前,病例组的NAA/(Cho+Cr)、NAA/Cr、NAA/Cho明显低于对照组(P<0.01);治疗 后,病例组的NAA/(Cho+Cr)、NAA/Cr、NAA/Cho明显高于治疗前(P<0.01),仍低于对照组(P<0.01);而3 组间 Cho/Cr 相比差异无统计学意义(P>0.05)。病例组治疗后海马 NAA/(Cho +Cr)与 PANSS 总分、 HAMD-24 分值显著相关(P<0.05),NAA/Cho 与 PANSS 总分、NAA/Cr 与 HAMD-24 分值显著相关(P< 0.05)。结论:IPT可以改善抑郁症患者抑郁程度,其机制可能与调节海马区代谢物的水平有关。
英文摘要:
      To evaluate the role of interpersonal psychological therapy (IPT) in antidepressant therapy and explore its mechanism using 1 H-MRS. Methods: Seventeen patients with depression were included in the patient group, and 17 healthy volunteers who underwent physical examination were included as the control group. Patients with depression were treated with IPT for 4 weeks. The positive and negative symptoms scale (PANSS) and Hamilton Depression Scale (HAMD-24) were employed before treatment and after 28 days of treatment to evaluate efficacy. 1 H-MRS of the hypothalamus was performed to analyze and compare the N-acetyl aspartic acid (NAA)/[choline complex (Cho) + creatine (Cr)], NAA/Cr, NAA/Cho, and Cho/Cr ratios in the 2 groups before and after treatment. Results: In patients, PANSS negative symptoms, general condition, total score, and HAMD-24 score were significantly decreased after treatment (P<0.01), and PANSS positive symptoms were not statistically significant (P>0.05). The NAA/(Cho + Cr), NAA/Cr, and NAA/Cho in the patient group before treatment were significantly lower than those of the control group (P<0.01). After treatment, patient group NAA/(Cho+Cr), NAA/Cr, and NAA/Cho were significantly higher than those before treatment (P<0.01) though still lower than those of the control group (P<0.01). There was no statistically significant difference in Cho/Cr between the three groups (P>0.05). NAA/(Cho+CR) was significantly correlated with the PANSS total score and HAMD-24 score (P<0.05); NAA/Cho was significantly correlated with the PANSS total score; and NAA/Cr was significantly correlated with the HAMD-24 score (P<0.05). Conclusion: IPT can improve the severity of depression in patients, possibly by regulating the ratio of metabolites in the hippocampus.
