      目的:探讨干扰素-γ(IFN-γ)、肿瘤坏死因子-α(TNF-α)、降钙素原(PCT)和C-反应蛋白(CRP)的含量 变化在细菌性脑膜炎(BM)早期诊断中的意义。方法:选取BM患者30例为BM组,病毒性脑膜炎(VM)患 者30例为VM组,健康体检者30例为健康对照组。检测BM组和VM组脑脊液中IFN-γ、TNF-α、PCT和CRP 含量,3组血液中IFN-γ、TNF-α、PCT和CRP含量。结果:与VM组相比,BM组患者脑脊液IFN-γ含量增高, TNF-α、PCT 和 CRP 含量升高(P<0.05)。与健康对照组比较,BM 组和 VM 组患者血液 IFN-γ含量降低, TNF-α、PCT和CRP含量升高(P<0.05);与VM组比较,BM组患者血液IFN-γ含量降低,TNF-α、PCT和CRP 含量升高(P<0.05)。IFN-γ、TNF-α、PCT和CRP指标联合检测率、准确性和特异性显著优于各指标的单独检 测。结论:IFN-γ、TNF-α、PCT和CRP的联合检测在BM早期诊断中具有重要的临床意义。

      To investigate the effects of differing levels of interferon-γ (IFN-γ), tumor necrosis factor-alpha (TNF-α), procalcitonin (PCT), and C-reactive protein (CRP) in the early diagnosis of bacterial meningitis (BM). Methods: A total of 30 patients with bacterial meningitis were recruited as the BM group, and 30 patients with viral meningitis (VM) were assigned to the VM group. An additional 30 healthy patients were selected as the control group. The cerebrospinal fluid levels of IFN-γ, TNF-α, PCT, and CRP were measured in BM and VM groups, and blood levels of IFN-γ, TNF-α, PCT, and CRP were measured in all 3 groups. Results: Compared to the VM group, patients in the BM group showed increased cerebrospinal fluid IFN-γ content and increased TNF-α, PCT, and CRP content (P<0.05). Compared with the control group, patients in the BM group and the VM group showed decreased blood IFN-γ content and increased blood TNF-α, PCT, and CRP content (P< 0.05). Compared with the VM group, patients in the BM group displayed decreased blood IFN-γ content and increased blood TNF-α, PCT, and CRP content (P<0.05). The detection rate, accuracy and specificity of the combined assessment of IFN-γ, TNF-α, PCT, and CRP were significantly better than those of each index considered alone. Conclusion: Combined detection of IFN-γ, TNF-α, PCT, and CRP in the early diagnosis of bacterial meningitis has important clinical significance.
